Admissions
Requirements
All candidates are required to submit the following documents:
- Completed application form
- Completed affidavit of citizenship
- A copy of the candidate's High School Diploma or its equivalent, and a copy of the relevant school documents such as transcripts of grades - all in the candidate's native language certified by candidate's school or by a public notary
- An English translation of the above original documents, translated and
certified by candidate's school or by a sworn translator
- A good quality photocopy of the applicant's passport (original to be shown during the interview)
- Three recent 35x45mm photographs signed on the back in which the applicant's head is not covered
- A copy of the applicant's birth certificate (original to be shown during the interview)
- Supplementary material (support letters) helps to make positive decisions
- Certificate of proficiency in English (non-native speakers of English only)
- Medical Health Certificate completed by your doctor (including MRSA, HBV, HCV, HIV tests as well as chest X-ray report will be requested upon acceptance)
Furthermore, any candidates that are accepted for studies will be required to submit the following additional documents:
- Original High School Diploma or its equivalent legalized or with apostille
- Documentation confirming the payment of admission and orientation fees (see item #7 in Application Procedure below)
- Signed copy of agreement
IMPORTANT: Please be aware that a valid Polish student visa is required for all non-Polish citizens studying at a Polish university. For further information, please contact a Polish embassy or consulate in the USA or Canada as appropriate.
Applications must be accompanied by a processing fee of $150 (make checks out to "Jaroslaw Krejza")

Application Procedure
- Submit the application package to the Overseas Admissions Office.
- The office will check the completeness of the application, and will notify the candidate of the date of the test and the interview.
- The office will notify the candidate of the admission status: accept, reject or under consideration. The best candidates are accepted early and the decisions are made within 4 weeks.
- The accepted candidate will receive the certificate of enrollment and the contract to sign.
- The accepted candidates are required to mail the signed contract back and a deposit of $1000 not later than 4 weeks after receiving the contract.
- The deposit will be returned to the candidate before December 1st of the first academic year. However, if the candidate does not start the study, the deposit will not be refunded.
- The candidate is required to pay an admission fee of 2000 PLN and 1000 PLN orientation week fee before August 15, 2010.
- Tuition has to be paid not later than by November 15th, 2010 (act promptly to get financial aid).

Admission Deadlines:
There deadline for admission is the end of August.
However, due to the limited number of seats and the fact that the best candidates are accepted instantly we strongly encourage candidates to apply as soon as possible.
Please be aware that we reserve the right to change the deadlines.
